Pershing Health System (PHS) invites community members in Linn County and the surrounding area to participate in the 2024-2025 Community Health Needs Assessment (CHNA) Survey. This survey provides residents with the opportunity to share their experiences, concerns, and insights on health issues and access to care in their area. The feedback collected will be used to prioritize health initiatives, address gaps in services, and ensure PHS and local providers align services with the needs of the community.
Why Your Input Matters
By gathering input directly from residents, PHS can identify pressing public health concerns such as chronic illnesses, mental health, access to preventive care, and socio-economic barriers to healthcare. Results from the CHNA survey will also contribute to improving the availability and quality of health services, ensuring that future programs and partnerships reflect the community’s unique needs.
Examples of how survey feedback has previously shaped services:
• Development of local mental health resources
• Expanded chronic disease management programs
• Introduction of public health education initiatives
How the Data Will Be Used
The survey findings will be compiled into a Community Health Needs Assessment report, which will be publicly available in mid-2025. This report will serve as a planning tool for PHS and public health agencies to create strategies that:
• Expand access to care for underserved populations.
• Improve health education and awareness.
• Build partnerships with local organizations to better address critical needs.
PHS is also committed to identifying service gaps and working toward solutions that promote healthier lifestyles and better outcomes for all residents.
Participation
Participants can complete the survey by Jan. 31 online at https://phsmo.org/resources/community-health-needs-assessment or in paper format at any of the following locations: Pershing Health System, Brookfield Public Library, Marceline Public Library, Linn County Health Department.
By completing the survey participants will have the chance to enter a drawing to win one of two $150 gift cards.

Compliance with ACA and Community Accountability
Nonprofit hospitals, including PHS, are required under the Affordable Care Act (ACA) to conduct a community health needs assessment every three years. The assessment process helps hospitals:
• Ensure that public health strategies remain responsive to community needs.
• Promote transparency and accountability in health care delivery.
• Align hospital services with long-term health improvement goals.
